WHAT IS PROSTATE BIOPSY?


A prostate biopsy is a procedure that involves taking a few tissue samples from the prostate for examination under the microscope. This is the only conclusive way to diagnose or rule out prostate cancer.

    Your urologist may recommend a prostate biopsy for any of the following reasons:

  • Your PSA level is elevated above what is normal for your age and prostate size
  • There was a nodule or lump felt on your prostate during examination
  • An MRI of the prostate shows an area suspicious for cancer
  • Your previous biopsy was normal however the PSA continues to rise significantly



THE PROCEDURE

How do you prepare for a biopsy?

  • The doctor may recommend certain blood tests to assess the bleeding risk
  • A urine sample may be needed to look for infection
  • Medicines which increase the risk of bleeding need to be stopped for 5-7 days prior to the biopsy.
  • Antibiotic tablets are prescribed from 2-3 days before the procedure
How is a prostate biopsy performed?

  • You will be asked to lie on your left side with your knees pulled up to the chest.
  • Sedation is not needed for most cases.
  • An ultrasound device is inserted via the rectum (trans-rectal ultrasound).
  • Local anesthetic will be injected around the prostate to minimize pain.
  • Biopsies are taken using a spring-loaded needle gun removing thin slices of the prostate.
  • You will be aware of the entire procedure going on, however pain is usually minimal.
  • About 12-15 samples are taken and the entire procedure takes 15-30 minutes.
  • You will be observed in the ultrasound room for a few minutes to make sure there is no bleeding
  • You will be allowed to go home once you have passed urine.
What to expect after the biopsy?

  • You will be prescribed some antibiotics and painkillers for the next 2-3 days
  • Avoid strenuous activity.
  • You will feel some soreness in the rectum and may notice blood in the stools or urine which generally clears up quickly.
  • Please contact your doctor if you develop fever, inability of pass urine, persistent bleeding or clots and worsening pain.

INFECTION
This occurs in 4-5% of patients after a biopsy and about 1-2% of patients may need to be admitted to hospital for antibiotic injections. Precautions, including giving you antibiotics before the procedure, are taken in order to minimize this risk.
BLEEDING FROM THE RECTUM
Bleeding from the rectum is common and usually stops in 3-7 days.
BLEEDING IN THE URINE
Bleeding in the urine is quite rare and generally minor and short-lasting.
BLEEDING IN THE SEMEN
Some patients have difficulty passing urine following a biopsy and may need a catheter temporarily.

FAQs

Who requires a biopsy?

If your PSA is elevated or rising or if your urologist felt a nodule on your prostate, you may need a biopsy.

Is the biopsy painful?

Local anesthesia is injected around the prostate to minimize the pain. You will be aware of the entire process but pain should be minimal.

What percentage of biopsies are cancer?

This depends on the PSA level. Overall about 15-25% of biopsies are positive for cancer.

How long does it take to recover?

You will be home in a couple of hours and back to normal activity in 2-3 days.
